Below are samples of statements faculty can use or adapt on their syllabi. 

Sample 1

Â鶹ÊÓƵ is committed to combating power-based personal violence. As a result, you should know that university faculty and staff are required to notify the Title IX coordinator of any instances of gender/sexual harassment that may be a violation of policy- including sexual assault, relationship violence, stalking and sexual harassment.  Sharing this information ensures that those harmed are provided appropriate resources and support options. What this means is that as your professor, I may be required to report an incident of sexual misconduct that is directly disclosed to me, or of which I am somehow made aware.

Sample 2

Â鶹ÊÓƵ is committed to fostering a safe, productive learning environment. As an instructor, one of my responsibilities is to help create a safe learning environment in our class. Â鶹ÊÓƵ’s (and federal Title IX law) policy prohibit discrimination on the basis of sex, which includes sexual misconduct — (sexual harassment, domestic and dating violence, sexual assault, and stalking.) We understand that sexual violence can undermine students’ academic success and we encourage students who have experienced some form of sexual misconduct to talk to someone about their experience, so they can get the support they need.

It is my goal that you feel able to share information related to your life experiences in classroom discussions, in your written work, and in our one-on-one meetings. I will seek to keep information you share private to the greatest extent possible. However, I also have a responsibility to notify the Title IX Coordinator when I become aware of incidents of sexual misconduct that could be a violation of policy.

Sample 3

In the event that you choose to write or speak about having survived sexualized violence, including rape, sexual assault, dating violence, domestic violence or stalking, Â鶹ÊÓƵ requires that, as your instructor, I share this information, if it is a potential policy violation, with our Title IX department. Someone from Title IX may contact you to let you know about support measures as well as options for holding accountable the person who harmed you.

If you do not want Title IX notified, instead of disclosing this information to your instructor, you can speak privately or confidentially with the following people on campus and in the community.
