Types of Forms and Requests

Learn how to submit various form types and check the status of previously submitted forms by reviewing the form types and their instructions below.

Dynamic Form

Dynamic forms are completed electronically through a direct link, NOT manually (PDF).

  • Read instructions carefully on the landing page of the Dynamic form you are completing.  
  • Have any required documentation ready to attach directly to the Dynamic form.
  • If the form you are completing is a Dynamic form, you DO NOT need to submit it anywhere else unless it states otherwise.  

Dynamic Forms will be automatically routed and processed.

How Do I Check The Status of a Dynamic Form?

Need to check the status of your submitted dynamic form? You can check back  to view your pending/draft Dynamic form and to review any of your completed Dynamic forms.

I Do Not Have a Â鶹ÊÓƵ Email Address, How Do I Submit a Dynamic Form?

If you are a non-current student, an account will need to be created on the Dynamic Forms site using a personal email address.

PDF

Most PDF forms are completed manually and submitted by using the "Submit Documents" link on our contact us page. Please review the instructions on your forms/documents for details on whether it is required to be returned in person or via postal mail due to a wet signature.

I am Required to Submit My Form In Person, Where Do I Go?

If the form requires in-person submission,  you will need to schedule an  appointment at the Kent Campus or contact a regional campus location to inquire about their hours of operation.

I am Required to Submit my Document with a Wet Signature. What are my Submission Options?

You have the option to submit forms/documents in person using our drop box located on the first floor of the Library at the Kent Campus or at any regional campus location. Additionally, you can choose to submit them via postal mail.

How Do I Scan a Document Using My Mobile Device?

Instructions for Android Users

How To Scan a document: 

  1. Open the Google Drive app. 
  2. In the bottom right, tap "Add." 
  3. Tap "Scan." 
  4. Take a photo of the document you'd like to scan. "Tap 
    1. Adjust scan area: Tap "Crop." 
    2. Take photo again: Tap "Re-scan current page "
    3. Scan another page: Tap "Add" 
  5. To save the finished document, rename the document and tap "Save" or "Done". 

Add a scanning shortcut to your Home screen:

  1. Open your Android phone or tablet’s widgets. 
  2. Find the "Drive scan" widget. 
  3. Touch and hold the widget. 
  4. Drag it onto your Home screen. You may be asked to select an account. 
  5. Choose the folder you’ll save documents inside. If you want to create a folder, tap New Folder. 
  6. Tap Select. You’ll see the folder name in the widget.
 

Instructions for iOS Users

How To Scan a document: 

  1.  Open a note or create a new note. 
  2. Tap the camera icon, then tap "Scan Documents". 
  3. Place your document in view of the camera on your device. 
  4. If your device is in Auto mode, your document will be automatically scanned. If you need to manually capture a scan, tap the round photo capture button or one of the Volume buttons. 
  5. Drag the corners to adjust the scan to fit the page, then tap "Keep Scan". 
  6. You can add additional scans to the document or tap "Save" when you're done. 
  7. Learn how to use the to scan notes from your iPhone or iPad to your Mac.
 

FlashLine

Numerous forms and requests can be conveniently completed when you log into . Remember, students are expected to utilize their individual FlashLine accounts for form submissions. Sharing FlashLine account details, including with family members, is strictly prohibited according to University policy.

On occasion, you may be prompted to log into your Â鶹ÊÓƵ Applicant Portal to submit a form. This is the same platform that you used to submit your application to Â鶹ÊÓƵ at apply.kent.edu.
