Findings from research undertaken by researchers with the Center for Public Policy and Health, Akron Children's Hospital, and the Pathways Community HUB Institute (PCHI) are included as a chapter in the book Aligning Systems for Health: Two Years of Learning, released by the Georgia Health Policy Center. The volume can be downloaded
The chapter overviews research featuring an assessment of 28 PCHI certification prerequisites and standards to determine whether the Pathways Community HUB model fosters aligning across healthcare, social service, and public health sectors. It also overviews an analysis of data from four organizations implementing the HUB services in three communities in Ohio and Michigan in order to understand how development of HUB networks enables cross sector alignment. A full research brief is also avaiable for download .
Funding for this research was provided by the Robert Wood Johnson Foundation and the Georgia Health Policy Center.
